JPS Health Network - Eligibility & Enrollment Center offers four JPS Connection programs for those who qualify: JPS Connection, ​​​​JPS CARES Program, JPS Connection Supplemental to Medicare, and JPS Connection Supplemental to Insurance. Due to the current restrictions, JPS Connection applications will only be accepted by mail, fax, drop-off or email. Eligibility Centers in Fort Worth and Arlington are the current drop-off locations. Eligibility Enrollment phone lines are open Monday – Friday, 8:00 AM – 4:30 PM for questions or inquiries regarding JPS Connection.
