JPS Connection membership is only valid at JPS Health Network locations. Due to current restrictions, JPS Connection applications will only be accepted by mail, fax, drop-off or email. Eligibility Centers in Fort Worth and Arlington are the current drop-off locations. Eligibility Enrollment phone lines are open Monday – Friday, 8:00 AM – 4:30 PM for questions or inquiries regarding JPS Connection. Four JPS Connection programs are available for those who qualify: JPS Connection provides assistance to patients without health insurance; JPS CARES Program provides assistance to patients without health insurance and experiencing homelessness; JPS Connection Supplemental to Medicare provides assistance to patients with Medicare Part A&B or a Medicare Plan contracted with JPS Health Network; JPS Connection Supplemental to Insurance provides assistance to patients with a primary insurance plan that is contracted with JPS Health Network.
